You provide certain information about yourself or your property when you receive a quote, buy or renew a policy or make policy changes. We confirm or supplement this with information from various consumer-reporting agencies that provide consumer reports, such as motor vehicle reports, claim reports and credit reports. When we underwrite your insurance based on this information, it may
result in an increased premium or other adverse action. It may also improve your insurance rate.
How did the information affect my insurance rate?

What should I do if I have a question?
If you have a question about the driving history information, please contact <vendor>. You may obtain a free copy of your MVR or CLUE report for 60 days after you receive this notice. Also, you may dispute with <vendor> the report’s accuracy or completeness.

If your question concerns your credit report, please obtain a copy of it from <vendor> and verify its accuracy before calling us. For 60 days after you receive this notice, you may obtain a free copy of your credit report. Also, you may dispute with <vendor> the report’s accuracy or completeness.
A consumer-reporting agency did not make the decision to take the adverse action and cannot explain why the action was taken. If the information in a report is incorrect, you may call Policy Services for a review of your rate after the report has been corrected by the consumer-reporting agency.
How can I get more information?
For 90 days after we send this notice, you may obtain in writing the specific information supporting our reasons for this action, if the information is not stated above or protected from disclosure by law. You may also:
- learn about and access recorded information about you;
- request correction of the information and reconsideration of any underwriting decision based on incorrect information;
- file a statement setting forth what you think is the correct information, and why you disagree with any refusal to correct the information;
- learn the identity of others to whom we may have disclosed this information in the previous 2 years.
To do so, send a written request to Policy Services, <address varies based on processing site> describing the kind of information you want to review. Include your full name, address, policy number (if applicable), and either your date of birth, social security number or driver’s license number.
